Flood/Damage to Property Information Needed

Attention All LDAF Employees:
SUPERVISORS: We need your immediate assistance with obtaining this information. Not all employees receive emails on mobile devices so please phone or text employees under your supervision to obtain the requested information.
As our focus begins to shift to repair and restoration in certain areas, Commissioner Strain wants to know which employees received damage to their homes and property. Of those employees directly impacted, we need to know which ones have flood insurance. This information will allow us to focus our resources and provide assistance to our employees who need it the most.
In addition, if you have suffered damage and have been displaced by the recent floods, it is imperative that you sign up for disaster assistance at FEMA’s website, DisasterAssistance.gov. It is imperative that you do this immediately. When you register, you will be given a FEMA identification number which you should email Human Resources Director Melissa Sylvia at msylvia@ldaf.state.la.us once obtained.
Please provide the information regarding damage to property and insurance if any, city of residence and FEMA identification number directly to Human Resources Director Melissa Sylvia at msylvia@ldaf.state.la.us.

- Information Checklist
Before you start your application, please have the information below and a pen and paper ready.
Social Security Number
You, another adult member or minor child in your household must have a social security number. You must also be a U.S. citizen, non-citizen national, or qualified alien.
If you don’t have a number, read How do I apply for a new or replacement Social Security number card, You can get instructions and find out what documents you need.
Once you have your number, you can come back toDisasterAssistance.gov or call FEMA at one of the phone numbers above to apply.
Insurance Information
Describe the type(s) of insurance coverage you have. This could include coverage under policies like homeowners, flood, automobile, or mobile home insurance.
Damage Information
Describe the damage caused by the disaster. Include the type of disaster (like flood, hurricane, or earthquake) and type of dwelling or vehicle (like mobile home or house, or car or boat).
Financial Information
Provide your total annual household income, before taxes, at the time of the disaster.
Contact Information
Provide the address and phone number of the property where the damage happened, and the address and phone number of where we can reach you now.
Direct Deposit Information (optional)
If approved, we can deposit your funds directly into your bank account. You just need to provide the following banking information:
- Bank name
- Type of account (like checking or savings)
- Routing number
- Account number
